Family Partner

The Family Partner Program works together for families in Southwest Virginia to help support each individual family in service development according to their unique needs.
· Families require services and supports
  reflecting their race, ethnicity, gender
  orientation, language, socio-economic
  background, and family structure
· Families have access to information and
  training
· The needs identified by families as most
  important will determine services
· Families share power to make decisions and
  responsibility for outcomes
· Families and their system partners know their
  individual strengths, limitations, and fears
· Families and their organizations get both
  respect and protection from their system
  partners

Family Partner Program
Education, training, and information are provided to parents with children in child-serving systems in Planning District One area counties of Lee, Wise, Scott and the City of Norton.

What services does the Family Partner Program with Frontier Health provide?
· Serves as a mentor for parents
· Provides referral and resource information
· Assist parents in building a support system for
  themselves and their children
· Advocates for parents and their children
· Serves as a bridge between child serving systems and families

Who qualifies?
Parent of child involved in any of the following child serving systems:
· Mental Health
· Mental Retardation
· Child and Youth Services
· Juvenile Probation
· Drug and Alcohol
· Special Education

Who is the Family Partner?
The Family Partner is a parent who has personal experience with services and agencies that serve children.


Advocacy & Affiliation

The Family Partner Program has access to resources to make your experience more family-friendly. We are here to advocate for your family. We are also affiliated with the local chapter of the Federation of Families for Children’s Mental Health. Please call (276) 431-4370, for more information.

Systems of Care
   Family Partner is in place to support the values and principles of Systems of Care.
The core values of Systems of Care are:

  driven
· Community-based
· Culturally competent and responsive

   The core principles of Systems of Care provide for:
· Service coordination or case management
· Prevention, early identification, and
  intervention
· Human rights protection and advocacy
· Non-discrimination in access to services
· A comprehensive array of services
· Individualized service planning
· Services in the least-restrictive environment
· Family participation in ALL aspects of planning,
  service delivery, and evaluation

Working Together for Families
   Families are the best advocates for children struggling with special needs. The Family Partner Program focuses on assisting families in helping their children to achieve their full potential. By building strong partnerships with families, schools, and professionals, children and families can reach their goals. The Family Partner Program encourages active involvement of the family in meeting the child’s goals.


Resources

   Support groups are available for families with children with mental health, behavioral, mental retardation, or developmental issues. Monthly meetings are held at various locations throughout the LENOWISCO district. For more information or to schedule group trainings, contact the Family Partner Program. The Family Partner program also offers resources through its Family Lending Library, Resource Lists, and other support information.
