TRANSITIONS is Helping today's teens become tomorrow's adults. The TRANSITIONS teaches independent living skills fo 16- to 21-year-olds so they may achieve independence in the most appropriate, least-restrictive environment in Lee, Wise and Scott Counties and the city of Norton.
