Intellectual and Developmental Disabilities

People “have” intellectual and developmental disabilities — they are not “defined” by them. Frontier Health’s caring, trained staff provide support so individuals can take steps to achieve their personal level of independence. They learn to make their choices, find friends who understand them, work and earn money, exercise their rights and become an important member in their community.

OPENING DOORS

Frontier Health opens doors to challenging and rewarding opportunities for people with disabilities through facility and community case management, and employment services. Vocational training includes pre-employment preparation, job skills training, job counseling, on-the-job training, follow-up support, and competitive employment. Life skills training is provided for social, community, and independent living.

SUPPORTED LIVING

Supported Living Homes provide housing and services to adults with intellectual and/or developmental disabilities who have attained most of the necessary skills for semi-independent and supported living.

Frontier Industries offers vocational, employment, residential, developmental and community integration services to individuals who have intellectual, developmental, emotional, and/or physical disabilities. The program offers opportunities for individuals with severe disabilities to learn and master life skills necessary to achieve independence and to live and/or work in the community of their choice. Skills include achieving personal goals, developing daily living skills, building job skills, improving personal awareness and self-esteem, and practicing social skills.

Developmental Services provides a continuum of services to children, youth and adults with intellectual or developmental challenges and disabilities including case management, day support services, residential support, and community engagement. All services provided are referred through Case Management. Day Support Services are provided through the Adult Activity Center five days a week. Transportation can be arranged. REFERRAL

All services provided through Developmental Services are referred through Case Management. People with intellectual or developmental challenges and disabilities are eligible for Case Management.

Each individual has a case manager, who is available to assist the individual with linkages to other services available to them. The case manager also works closely with other agencies to ensure the individual has access to a full continuum of care.

ADULT ACTIVITY CENTER

Day Support Services are provided through the Adult Activity Center five days a week. Transportation can be arranged. Referrals to Day Support are made through Case Management. The Adult Activity Center provides training for daily living activities including grooming, housekeeping, shopping, cooking and basic self-care skills.

Staff work with individuals and in small groups to help each person acquire the skills necessary for community living. The program also focuses on community-centered activities.

Residential Support Services offers individuals training, assistance and supervision within their own homes to avoid living in an institution. Staff provide training in cooking, cleaning, shopping, behavior management, social skills and medication management. Services may be provided seven days a week during the morning and evening hours.

Congregate Residential Support Services are also available for people with intellectual or developmental challenges and disabilities. These services are provided in a Group Home setting and offers the same training and supports as identified above.

Independence Unlimited, a fully integrated training facility for individuals with disabilities, fulfills contracts for many different area businesses. Products and services include floor care, landscaping, cleaning, first-aid supplies, medical kits, personal protection equipment and wiping cloths. Independence Unlimited offers these services competitively and with high quality standards.

Independence Unlimited opens doors to challenging and rewarding job opportunities for people with disabilities. Individuals develop marketable job skills that may lead to employment in the community.

WORK ADJUSTMENT SERVICES

Work Adjustment Services prepares the employee for placement in Supported Employment or Community Employment. The purpose of Work Adjustment is to sharpen existing skills that help individuals prepare for community employment. Work Adjustment Services Community-based psychosocial rehabilitation program is provided for people with serious or persistent mental illnesses.
