

HEALTHY TRANSITIONS
Healthy Transitions is a free, state grant funded program designed to assist young adults aged 16-25 who reside in Greene County, Tennessee, and are struggling with mental illness, severe emotional disturbances, intellectual disabilities, substance abuse and/or co-occurring disorders. We offer therapy, peer support, case management, education assistance and employment resources.

PEER GROUPS
We hold peer groups every Tuesday from 2-4pm and on Thursday from 2-5pm. Our peer groups are designed to help young adults learn how to cope with mental illness and to learn new daily living skills.
Young Adult Leadership Council is held every third Thursday each month during our peer group. YALC is a peer focused group. This gives our peers the opportunity to speak about current issues and what changes they want to see happen.
What is YALC?
This is a youth-driven council that initiates leaders and empowers young adults (age 16-30) to use their voice, to bring awareness and inspire change in the difficulties youth of today face.
